By completing this form, you appoint Sizwim Homecare as your care provider. It outlines our responsibilities to you, the services we’ll provide, and your role in ensuring we deliver the best care possible.

Our Commitment to You:

  • Care Needs Assessment – Provided within 24 hours for emergencies or 48 hours for non-emergencies after we receive all necessary details.
  • Personalized Care Plan – Created and agreed with you within 48 hours of your assessment.
  • Outcome Agreement – We’ll discuss and agree on your expected care goals with your consent.
  • Reliable Visits – We’ll arrive at scheduled times, and we ask for 48 hours’ notice if plans change.
  • Home Diary – We’ll maintain a care diary in your home (with your permission).
  • Annual Review – We’ll check in yearly (or more often if needed) to adjust care as required.
  • Professional Coordination – We’ll work with other healthcare professionals involved in your care.
  • Ongoing Support – We’ll monitor and adjust your care plan as needed, with your input.

Your Responsibilities:

  • Access – Allow our team into your home at agreed times.
  • Communication – Inform us if your needs change.
  • Notice – Give us 48 hours’ notice for visit cancellations or new arrangements.
  • Fees – Pay for services as per our agreed terms.

Care is billed at an hourly rate—contact us for local pricing.
